About our post school pathways

Adulthood doesn’t begin at 18. It emerges gradually, and with the right support, it blooms.

Our post-school services are designed to accompany individuals as they navigate this transition, helping them grow from a place of security into a space of possibility. We start from the school-leaving age and walk alongside them as they carve out adult lives that feel authentic, joyful, and self-directed.

Every adult has unique aspirations. Some dream of working as independently as possible. Others want to live independently, form relationships, or contribute to their community. We help individuals identify those goals and build the skills to reach them—at their own pace.

Self-advocacy is central. We support individuals to understand their rights, express their preferences, and make informed choices. Communication tools, sensory strategies, and access to assistive tech are integrated into support plans wherever needed.

We also focus on the quieter aspects of transition: the need for routine, the anxiety of unstructured time, the emotional impact of moving on from childhood systems. Through creative planning, group programmes, and trusted relationships, we help ease these growing pains.

Adulthood is not just about surviving, it’s about thriving, connecting, and shaping one’s own path. That’s the journey we’re proud to support.



What might this look like?

A fictional example (intensive) based on real interventions:

A New Chapter: How Pebble Pathways Supported Daniel’s Independence

Daniel, a 29-year-old with autism, had recently moved into supported living after years with family. While he enjoyed his new space, his team noticed growing frustration when trying to express himself and complete everyday tasks. He often shut down during group activities and resisted scheduled responsibilities like meal prep or laundry.

Step One: Understanding the Individual

During our first 2.5-hour session, we spent time with Daniel in his apartment. We observed his routines, spoke with his support workers, and engaged in low-pressure conversation to build rapport. Daniel typed short messages on his phone when speech became too difficult. Together, we identified a key goal: improve Daniel’s ability to communicate, manage his stress, and participate in daily tasks more confidently.

Step Two: Implementing a Tailored Intervention – Second 2.5-hour Visit

  • AAC Practice Using a Speech-Generating Device
    To reduce frustration, we introduced a user-friendly speech-generating app on Daniel’s tablet. We customised it with personal icons, food preferences, and routine phrases. Within the first session, Daniel used the app to request his favourite snack and comment on the weather. This breakthrough gave him a sense of autonomy and decreased communication-related anxiety.

  • Vocational Training Tasks
    We structured a task schedule based on Daniel’s interests, starting with organising household items and packing snack boxes for his day program. Using visual task cards, Daniel completed steps independently and took pride in his growing efficiency. These tasks not only boosted his confidence but helped prepare him for future part-time volunteer work.

  • Mindfulness and Relaxation Sessions
    To address Daniel’s tendency to become overwhelmed, we introduced short, guided breathing exercises using a visual timer and calming app. He responded well to 5-minute sensory breaks with nature sounds and a weighted lap pad. These strategies were embedded into his day, helping Daniel manage transitions with greater ease.

Follow-Up Support

Two additional 2.5-hour visits helped embed these strategies and adapt them to Daniel’s evolving needs. His support staff were trained in using the AAC device consistently, and we developed a mindfulness “menu” for Daniel to choose calming activities throughout his day.

Outcome

Daniel now communicates more effectively and confidently participates in daily living routines. His support team has reported a noticeable drop in shutdown episodes, and Daniel proudly uses his device to greet visitors and ask questions around his interests. With practical tools and structured encouragement, Daniel is carving out a more independent, expressive life.
